It has been an honor and pleasure to have had the opportunity to be your physician. My 30+ years of medical practice have provided me with many wonderful experiences and much gratification. It is, therefore, with some trepidation, but great anticipation, that I am writing to inform you of my intention to retire as of May 24, 2018.
